The first nursing badge is free, the cost to replace a nursing badge will be $10

On-campus nursing students must come to the Razorback Card Office to create their nursing badge:

** Students must bring a physical government issued ID, such as a driver's license or a passport
** If fees occur, payment will be made in office, and must be done through credit/debit card 

Off-campus nursing students have the option to complete the online authentication process below:

** Follow the instructions here to authenticate your identity
** Once the badge has been processed, it will be shipped out to the mailing address listed on Workday

There is no cost to you if you have an authorization form signed by the department supervisor
1. You will need a physical government issued ID and your Universal ID number. If you do not have this number please consult with your supervisor.
2. Arrive to the Razorback Card office with your physical government ID (state, passport, military) and form filled out online.
3. We'll take your picture and print you a ID.

Faculty/Staff may be entitled to up to 2 replacements before a $25 fee

The first badge is paid by your Department.
*Parking and Transit has multiple versions of badges. When you arrive please know which one you need.*
1. You will need a physical government issued ID and your Universal ID number. If you do not have this number please consult with your supervisor.
2. Arrive to the Razorback Card office with your physical government ID (state, passport, military), Universal ID number, and the authorization form filled out prior to arrival.
3. We'll take your picture and print you a badge.

Replacements will cost $25

The first badge is paid by your Department.
1. You will need a physical government issued ID and your Universal ID number. If you do not have this number please consult with your supervisor.
2. Arrive to the Razorback Card office with your physical government ID (state, passport, military), Universal ID number, and the authorization form filled out prior to arrival.
3. We'll take your picture and print you a badge.
